A. FOR INCOMING PRESCHOOL & GRADE 1 STUDENTS

1. Fill up the Application Form available at http://bit.ly/wkenroll2021. *
Please only fill this form up ONCE. If you have already filled the form up, you are NOT required to do so again.
2. In the Application Form, you will be required to upload the following documents:
a. Photo/online copy of Birth Certificate (Philippines Statistics Authority (PSA))
b. Photo/online copy of previous grade level Report Card
3. After validation of your Application Form, you will be contacted by email to go through a mandatory online admission screening through Zoom.
*NOTE: If necessary, you may opt to fill up the application form at the office. Please also book an
appointment to do so by calling 0908 865 8946 / 049 8521 7492 during office hours. To ensure social distancing efforts, only visitors with an appointment will be allowed into the school. Only ONE
parent/guardian per student is allowed to be present for the appointment.
If payment is through online banking …
4. After successful screening, you will be instructed to make payment for tuition fees through online banking, to the following bank account:

5. Upon payment, please send a screenshot/proof of payment to whizkids.registrar@gmail.com.
We will reply with a confirmation for your reference.
6. Upon receiving the confirmation, please make an appointment to submit the following documents in person to the office:
a. Original copy of Birth Certificate (Philippines Statistics Authority (PSA))
b. Original copy of previous grade level Report Card (if applicable)
c. Learner’s Reference Number (LRN)
d. ECCD Checklist
